Entry Requirements
Learners must hold an NQF Level 3 qualification. In addition, they must have at least six months of operational experience within an organisational environment. This ensures that learners have the foundational knowledge and practical exposure required to benefit from the programme.

Purpose of the Programme
This skills programme aims to equip learners to function as Workplace Incident Investigation Team Members. After completing the programme, learners will actively participate as incident investigation team members within their respective workplaces or occupational contexts.
Workplace Incident Investigation Team Members play an important role in identifying the causes of workplace incidents. They participate in investigations in line with regulatory requirements as well as organisational policies and procedures. Furthermore, they contribute objectively and constructively to investigation outcomes. As a result, organisations can reduce or eliminate future workplace accidents and incidents.
Learning Outcomes
By the end of the programme, learners will be able to:
- Collect and safeguard evidence related to workplace incidents or accidents.
- Participate in developing appropriate recommendations to address investigation outcomes.
- Analyse incident information and help determine the causes of workplace accidents and incidents.
- Assist in drafting clear and accurate investigation reports.
- Contribute to the development of corrective and preventative measures to address identified causes.

Programme Duration and Assessment
The programme runs over two days and includes both theoretical and practical learning activities. Learners must complete all modules and demonstrate competence before they can attempt the Final Integrated Supervised Assessment (FISA).
The FISA includes both written and practical components that integrate the relevant Exit Level Outcomes. It lasts a maximum of three hours and carries a total of 100 marks. Learners must achieve at least 70% to be declared competent. In addition, assessors supervise all FISA sessions, and they record and monitor virtual assessments using cameras to ensure integrity.
During the FISA, learners will respond to a scenario that includes various questions and tasks. Through this process, learners must demonstrate competence in the following areas:
1. Apply essential methods, procedures and techniques relevant to the discipline of occupational incident and accident investigation to effectively collect, safeguard and submit a range of incident investigation evidence and facts for analysis by the investigation team.
2. Apply critical and analytical thinking processes and tools to effectively participate in evidence analysis to determine the causes of occupational accidents and incidents.
3. Adhere to organisational and professional ethical conduct codes to actively participate in developing recommendations to deal with the outcomes of incident investigations.
4. Apply produce, communicate and present information reliably and accurately when contributing to the creation of comprehensive incident and accident investigation reports.
